Orthopedic and spine patient navigator
Unique to Mercy is the orthopedic and spine patient navigator. The relationship between the navigator and the patient is to provide and assist in a seamless movement through the surgical continuum of care. Through each step of the process, our patient navigator, a trained nurse, guides patients and their families by offering case management and emotional and educational support.
The patient navigator assists patients and their families with every aspect of care from answering preoperative surgery questions to guiding and updating the family members on their loved one’s progression of the surgery. In addition, our patient navigator continues the relationship with the patient post discharge, with phone calls to review opportunities for future care and answer any additional questions.
